District Health Department No. 2 Registered Nurses manage and coordinate each client’s health care team. The first visit consists of an extensive health and physical assessment of the client’s status, providing a plan of treatment with client participation, and scheduling appropriate health care team members. Caring and professional nurses obtain ordered lab tests, administer medications, teach clients to be independent with their health care. Our nurses also provide wound care, assist with obtaining medical supplies and provide IV therapy. Our registered nurse also provide care for pediatric patients from birth to age 18.
Home Health Aides
District Health Department No. 2 Home Health Aides are invaluable support for the nursing staff and for clients who are recovering from an illness or injury. They are experienced, state certified aides who provide basic personal care to help increase client’s independence: baths, grooming, simple wound care, basic exercise programs, and assistance with activities of daily living (ADL’s). Our aides observe and report to the primary nurse any changes in an individual's condition.
Through a partnership with Pinnacle Rehabilitation, expert assistance is provided to individuals with prescribed exercise and/or rehabilitation regimens. Staff help and teach an individual to regain flexibility, movement, and mobility. Your first physical therapy visit would consist of a thorough physical evaluation and assessment of you plan of care. Physical therapists provide comprehensive equipment for the delivery of pain/edema relieving modalities, gait training, therapeutic home exercise programs, and muscle re-education to help increase range of motion (ROM), balance, and coordination. Instruction and training in proper transfer and ambulatory techniques. Our therapists will also help you with the acquisition of transfer/ambulatory assistive devices, prosthetics, orthotics, and wheelchairs. They will provide a complete assessment of the home environment for mobility requirements, safety, and make recommendations regarding adaptive equipment needs.
