Notification Forms (Well Permits)

The Environmental Health Regulations and Board of Health Policy for District Health Department #2 require that prior to construction of a new residential or Type III Public well, or other type of water supply, and prior to extensive repair to an existing well, a person must submit a Notification/Permit Form to DHD2, for approval.

The Notification/Permit Form must be purchased, completed and submitted 4 business days  prior to commencing construction of a water supply.  Persons may purchase a Notification/Permit Form at any office of District Health Department #2 or they may instruct their well drilling contractor to obtain a Notification/Permit Form on their behalf.

After submission of the Notification/Permit Form, a person and their well drilling contractor may begin construction of the well after four (4) business days unless there is a problem that necessitates a longer review. In these cases the health department will advise the contact person specified on the Notification/Permit Form of the nature of the review that is necessary.

All Type III Public Water Supplies must receive a pre-drilling site visit by health department staff to review the site and a final inspection to review the construction.
