Home Health Care

District Health Department # 2, a not-for-profit organization, is proud to offer Home Health Care services to our communities. Our Home Care Program is certified by the State and has earned the Michigan Award for Excellence. We diligently strive to provide quality professional care in your own home so that you may live in the best of health, with independence, a sense of self-worth, and dignity for as long as possible.

"Home Health Care", bringing the most cost effective, most preferred, and the highest quality care to the comfort and familiarity of your own home.

Home Health Care Services can be provided in a client's own home for homebound older adults, pediatrics, and anyone in our community in need of continued Skilled or Therapeutic care after an illness, injury, or surgery.  Our program offers skilled nursing care, physical therapy, speech therapy, occupational therapy, home health aide, and medical social worker.

To qualify for Home Health Care under participating health insurance plans, a client must currently be under a physician's care, be unable to leave their home independently, and in need of intermittent skilled care or therapy. 

Friends, family members, nurses, discharge planners, or social workers may call District Health Dept #2 and request an assessment. Or possibly you, yourself are the one in need. However the client's physician must agree to the need for care, in writing, prior to admission to our Home Care Program.  We have a registered nurse "on-call" 24 hrs a day/7 days a week. Our policy maintains that a nurse would contact the client within 24 hrs of the referral to arrange an initial visit for admission to Home Care Services.
