Northeast Health Plan

The Northeast Health Plan- Part B is a community sponsored health benefit program that serves uninsured adults in Alcona, Iosco, Ogemaw, and Oscoda counties.

Who is Eligible?

• Adults aged 19-65 (ages 19-21 must apply at DHS and be denied before becoming eligible for Northeast Health Plan)
• Have no health coverage
• Income qualification
• Live in Alcona, Iosco, Ogemaw, or Oscoda County
• U.S. Citizen

Member Services and Co-Payments

• $5 per Physicians Visit
• $10 per Specialist Visit
• $20 per Urgent Care Visit (emergency room NOT covered)
• $5 generic prescriptions/ $10 brand prescriptions (some prescriptions not covered)
• $5 for Radiologist (x-rays)
• Lab work also covered
• No coverage for hospital services

* If you state you have no income you must apply for benefits at your local DHS office. If you are denied benefits you can become eligible for the Northeast Health plan.
