General Summary: Under the supervision of the Community Health Director, participates in public health nurse service delivery in a variety of public health programs. The specific programs will change based on the needs of the department. Currently those programs are Children’s Special Health Care Services Case Management, Lead Poisoning Prevention, Maternal Infant Health, Immunizations, Family Planning, HIV Prevention, Syringe services, STI, WIC and Communicable Disease surveillance and prevention, and/or any other programs and duties assigned. Travel Required.

General Summary: Under the supervision of the Community Health Director, Health Educators work to  educate the community on various health related topics. This position may be subject to teaching youth in the classroom, writing press releases and other media releases, and serving as a community liaison with other community organizations.

General Summary: Under the supervision of the Community Health Director, serves as a secretary, with certification as a Community Health
Worker (CHW), assigned to assist clients and their families navigate and link to community services/resources, with the
goal of enhancing healthy outcomes. Responsible for receiving calls and walk-in clients and providing them with
information or referring them to the appropriate person or office. Performs a variety of clerical tasks in support of
departmental programs.

Health Resource Advocate

Starting Rate of Pay: $ 58,750.00/Annually paid Bi-weekly.

General Summary: The health resource advocate (HRA) will provide front-line support for COVID-19 testing and reporting, help school districts identify emerging COVID-related health concerns, and strengthen best health practices by providing consultation, prevention services, and appropriate public health strategies for the district. The HRA will help to access and address the greatest needs in school health.

A cornerstone of public health is to promote health through strategies driven by evidence to control the spread of disease. The pre-hire process will include an immunity assessment through DHD2’s Immunization Clinic to demonstrate immunity for MMR (Measles, Mumps, Rubella), Tdap (Tetanus, Diphtheria, and Pertussis), Varicella, and Hepatitis B through vaccination or documented disease.
