Fill out this confidential form. We will mail you a free package of 10 condoms, lubricant, and sexual health and STD testing information in an unmarked envelope. We can only mail condoms to addresses within Alcona, Iosco, Ogemaw or Oscoda Counties. Please note in cases of extreme temperatures (hot and cold) your condom order may be delayed. If you order after 3 pm on a Friday, condoms will not be mailed until the following Monday.
